The Food Establishment Inspection Report is a weekly report that appears Sundays. Information is taken from reports provided by the Bi-City-County Health District, and individual reports can be viewed at its website, http://amarillo.gov/departments/community-services/environmental-health/food-inspections. The district is now using a numerical grading system, with a score of 100 points being equivalent to zero demerits.
100 points – No major violation at the time of inspection
85 points – Considered an average score
70 points or below – Requires re-inspection within 24 hours
COS – Corrected on site during inspection

B – 80 to 89
C – 70 to 79
F – 69 and below

(A/94) Cassidy’s Polo Club, 6019 S.W. 45th Ave. Hand sink not properly supplied; ice machine needs to be cleaned and sanitized. COS. Food manager must register with city of Amarillo. To be corrected within 180 days.
(B/85) Chicken Express, 2106 Second Ave., Canyon. Towel and food debris in hand sink. COS. Bird feather in vents above cooking area (a $78 re-inspection fee has been added); food/grease build-up on foil and wax paper and boxes containing each; ice scoop holder dirty. To be corrected within three days. Wire rack holding bags for corn-on-the-cob is rusting and chipped; utensils for scooping bulk product stored in product (repeat violation); several flies in kitchen; lights in walk-in cooler and freezer need to be brighter; dirty liquid in bucket under table; grease on grease trap and ground (both areas need to be cleaned and sanitized); ice build-up in freezer. To be corrected within 90 days.
(A/90) El Patio Mexican Restaurant, 1410 W. US Highway 60, Canyon. Potatoes stored in rusting table. To be corrected within three days. Splash guard needed at hand sink; condensation line from walk-in cooler needs to be reconnected to prevent water draining onto floor. To be corrected within 10 days. Walk-in cooler condensation dripping onto food product; food product in walk-in cooler stored on floor; personal cell phone in food prep area. To be corrected within 90 days.
(A/90) Fazoli’s, 2512 S. Soncy Road. Several items out of date; ice machine must be cleaned and sanitized; food debris on clean dishes. COS. Seals needed on front doors and front side door to prevent entrance of pests; ceilings and shelves in equipment must be clean to sight and touch; food manager must be registered with city of Amarillo. To be corrected within 180 days.
(A/96) Khiva Shrine, 305 S.E. Fifth Ave. No certified food manager; coving behind oven needs to be properly sealed. To be corrected within 180 days.
(A/94) Palio’s Pizza Café, 3562 S. Soncy Road. Scoops improperly stored in ice machine; bowl used as scoop; ice bucket not stored inverted; reach-in cooler not at proper temperature. COS. Seal needed on back door to prevent entrance of pests; hand washing signs needed in women’s restroom. To be corrected within 180 days.
(B/83) Pei Wei Asian Diner, 3350 S. Soncy Road. Oranges for self service and coffee filters not covered; expired food items found; hot water nozzle and ice machines need to be cleaned and sanitized. Hand sink used for other purposes and not properly supplied; floors, walls, ceilings and equipment must be clean to sight and touch; ceiling tiles above walk-in cooler need to be replaced; plate used as scoop; dishes stacked wet; seal needed on back door to prevent entrance of pests; garbage and refuse area with grease on ground must be kept clean and maintained; food manager must be registered with city of Amarillo; lights in back by office must have light shield. (A/93) Petro Dunkin Donuts 307, 8500 E. I-40. Chemicals must not be stored on prep tables or in areas where they may spill onto wares. COS. Cell phone on prep table; ice bucket not stored inverted; single use straws must be individually wrapped to prevent cross contamination; handle on pull-out cold hold unit is damaged. To be corrected within 180 days.
(A/98) Petro Stopping Centers T S, 8500 E. I-40. Back door must be self-closing; all employee food must be separated from store items. To be corrected within 180 days.
(A/99) Tri State Baking, 6702 FM 1541. Floor tiles broken and cracked holding water. To be corrected within 90 days.
